Children baptized in another Christian faith: If your school aged child or teenager was baptized in the Protestant, Baptist, Pentecostal Holiness or other Christian tradition and you now wish to bring him or her into the Catholic faith, you must register your child in Formation. You will then receive information regarding an  information  session to be held in mid-Sept, for both the parent and child.  For more information, contact Lucille Pavlock in the parish office.

FIRST RECONCILIATION AND FIRST EUCHARIST

Second Grade Students: If your child is in second grade and attended formation classes in first grade with excellent attendance, requirements for the final year of preparation include:

• Regular attendance at Sunday liturgy
• Attend second grade Formation classes. Miss no more than two classes each semester.
• Parent(s)/child must attend an interview in the summer.
• Parent AND child must attend two Saturday morning retreats for Reconciliation and Eucharist. Dates published at time of registration for upcoming school year.
• Complete Reconciliation and Eucharist instruction materials with parents.
• Provide a copy of child’s Baptismal certificate.
• Complete a Sacrament Enrollment Form and Sacrament Commitment Form
• Parent completes an online video course "Full Participation of the Mass"

Note: If your second through fifth grade child did not attend Formation the previous year, they will need to enroll on their grade level this year in order to complete the first year of  faith formation. The following year they  will prepare and receive First Eucharist the following year.

Sixth Grade through High School: Requirements for children grades 6-12, who have been baptized Catholic, but have not received the sacraments of Reconciliation and Eucharist include Regular attendance at Sunday Liturgy plus the following: 

  Year One:

• Attend one year Basic Formation class prior to preparation for Reconciliation and Eucharist. Classes are held grades 6 – 8, or High School.
• Provide copy of child's Baptism certificate

  Year Two:

• Attend regular Formation class or Life Night as determined by grade level
• Miss no more than two classes per semester
• Attend two Reconciliation and Eucharist preparation sessions in the spring. See First Eucharist Prep. Dates link. 
• Parent and child must attend two Saturday morning retreats for Reconciliation and Eucharist. 
• Complete Reconciliation and Eucharist instruction materials with parents.
• Provide a copy of child's Baptismal Certificate.
• Complete a Sacrament Enrollment Form and Sacrament Commitment Form.

 CONFIRMATION

Confirmation is a gift of the Holy Spirit which confirms our belonging to Christ in Baptism and which calls us to be witnesses of Christ’s love for others. Contact Dcn. Joe Wright for more information.

High School Youth Requirements:

• Provide original Baptismal certificate of candidate. We will make a copy and initial at the office.
• Attend one year of High School faith formation before entering Confirmation class in grade 11 or 12. LIFE NIGHTS, Catholic High School or attending a High School formation class at another Catholic parish fulfills this requirement.
• Attend  two retreats and complete 30 service hours. These requirements can be fulfilled any time from ninth grade until Confirmation.
• During eleventh or twelfth grade, final preparation includes attending Life Nights and Confirmation sessions. Parent must attend parent meeting.
• Select a Confirmation sponsor (criteria giving to parents & candidates)
• Saint paper on the saint whose name they are selecting.
• Candidates must complete all homework assignments, attend an exit interview with a parent and turn in all paperwork.
